Become an Authorized Sales Rep.

Interested in offering the ActivaRx Prescription Drug Card to your existing or new customers, simply become an ActivaRx Drug Card authorized Sales Representative.

Enrollment Procedures

An ActivaRx Sales Representative may enroll members by completing the application attached to the sales brochure and collecting a check for the initial one-time enrollment fee of $20.00. On the application the member must agree to have the monthly fee of $6.00 deducted from their checking account (or savings account) and must include a voided check or a deposit slip that includes their account & routing numbers. Agents can also enroll members online once they have become an Authorized Sales Representative.
