How the Prescription Drug Card Works

This prescription discount card is available to anyone under age 65. ActivaRx members receive substantial discounts on their prescription drugs that have been arranged with over 60,000 participating retail pharmacies and with Walgreens the mail service pharmacy. The discounted pricing on prescription drugs will be the same at any of the participating retail pharmacies. For members using the mail service pharmacy the discounts on their prescription drugs will be greater.

This ActivaRx Discount Prescription Drug Plan is not an insurance program. Members pay the full discounted price on their prescription drugs.

How to use the Prescription Drug Card

• Sign up for the Prescription Drug Card and become a member.
• Simply take your ActivaRx Prescription Drug Card along with your prescriptions to any participating retail pharmacy and receive your discounts immediately.
• Walgreens the mail service pharmacy may be used by calling the toll free number shown on the ActivaRx Prescription Drug Card.

Enrollment Procedures

You may enroll in the ActivaRx Prescription Drug Plan by completing the form online. There is an initial $20 one-time enrollment fee per individual. Membership will become effective on the 1st day of the month following the month of enrollment. The monthly dues are $6.00 per individual. They are paid in advance by means of a pre-authorized debit to your checking or savings account. By submitting your enrollment form to ActivaRx, you are authorizing a monthly debit to be initiated in the amount of $6.00 and a one time enrollment fee of $20.00.

Upon Enrollment you will receive a Membership Certificate, which explains the ActivaRx Prescription Drug plan, and your personalized ActivaRx Prescription Drug Card. Your drug card will contain the appropriate 800 numbers for locating convenient retail pharmacies or for ordering from the mail service pharmacy.
